Forms
The Center for American History’s Research and Collections Division has a number of forms available in Adobe Acrobat® PDF format. In order to view these forms you must have Adobe Reader® (version 3.x or greater) installed on your computer. Adobe Reader is available to download for free from Adobe Systems Incorporated.
Once you have opened the form, print it, fill it out legibly (see below), and either mail it to Reference Services, or bring it when you visit in person (addresses). Please DO NOT e-mail the form to us, as the reference e-mail account does not accept attachments.
Notice Concerning Your Information: The Texas Public Information Act, with a few exceptions, gives you the right to be informed about the information that the University of Texas at Austin collects about you. It also gives you the right to request a copy of that information; and to have the University correct any of that information that is wrong. Several of the forms use very narrow margins. If you have problems with the edges of the document being "cut off" by your printer, select the "Shrink to fit" option in the Print dialog box. This option will proportionally reduce the document so that it will fit within the printable area for your printer.
If you encounter other printing problems, try downloading the latest version of Adobe Acrobat Reader®. If this does not help, consult your printer documentation or try Adobe’s Customer Support Web site. Please do not telephone the CAH Research and Collections Division or the UT Libraries for Adobe technical assistance.
