Photographic – Off Site
Illegible or incomplete forms will result in the cancellation of photoduplication orders.
1. Review the Photoduplication Price List in guiding your decision regarding format of duplication as well as cost of order. Texas sales tax of 8.25% is applicable to all Texas transactions. Patrons with Tax Exempt Status must complete the TEXAS SALES AND USE TAX EXEMPTION form 01-339. Note that payment in advance is required. Once you submit your order, the Photo Services Coordinator will contact you about payment arrangements by email. Do not send payment until the Photo Services Coordinator has contacted you.
2. Print legibly and fill out the Photograph Order Form, including box or call numbers, a precise description of the material to be photographed, and the type of photographic reproduction desired. Include all necessary contact information, as well as UT student or other UT Systems affiliation status and whether photographs will be picked up or mailed.
3. Indicate the digital image [di_##### or e_xx_####], copy negative [CN#####} or color transparency [CT####] number on the photo duplication worksheet if available. This number should appear in the credit line for a published image. If possible, attach a photocopy or printout of the item for verification.
4. For fastest service, scan completed forms and email them to: cah.photos@austin.utexas.edu.

5. After receiving both properly completed forms we will contact you by email with an invoice.
6. You may pay by Visa, Master Card or American Express over the phone (512-495-4555) or by mailing a check or money order made out to The University of Texas at Austin. Once payment has been received we will process your order. If payment is made by credit card over the phone we will include a receipt from both the cash register and card machine with your order.
7. Completion of orders averages between two and three weeks. Larger orders may take longer. If time is a concern, contact the Photo Services Coordinator directly at 512-495-4555 or cah.photos@austin.utexas.edu.
8. We send all orders within the United States by standard first class mail. Express mail is available upon request but an account number or credit card must be provided for this service. Patrons whose orders are to be mailed outside the United States must provide an express mail account or credit card number for express mail service.
9. Your approved Permission to Publish form will be returned to you with your order.
